Social care teams provide services for older people, people with physical, sensory, cognitive disabilities
and people with HIV and Aids. Community services teams provide services for people with learning
disabilities. Community mental health teams, working with health services, provide services for people
with mental health problems. Services can include: information and advice, personal care and practical
support at home, equipment and help with house adaptations, access to residential or nursing home care
and links with other organisations who may be able to help.

Helping people at home
Your assessment will begin within two calendar days of you making contact. At present this excludes
weekends. If we can assist you, we will discuss and agree this with you within 28 calendar days, and
where possible within two weeks of you first contacting us. You will receive a copy of your assessment in
an appropriate language and/or format. If we think that you are at risk of immediate harm we will respond
at once. We will involve you, whether you are a service user or a carer, in deciding how we will help you
and write how we plan to help you in a "care plan." We will give you a copy of the care plan. We will also
give you the name of the person responsible for the coordination of your care. We will review your care
plan with you, whether you are a service user or a carer, if your circumstances change and, at least,
annually. You will receive a copy of this review. Following an assessment, where possible we will include
a contingency plan in the event of something unexpected happening which affects your care. If practicable
we will enable you, whether you are a service user or a carer, to arrange your own care/support through
the use of direct payments and/or vouchers where these apply. We will provide equipment within seven
working days of your assessment. After your assessment is complete, we will provide services within 28
calendar days and where possible within two weeks. If you are a carer, we will also offer you an
opportunity to discuss separately what your needs are.
